Records Requests

You have a right to request your records. Please email me at Hannah@deeplyrootedtx.com for any records requests. I will reach out to your about further steps to receive records.

Complaints

I aim to provide the best level of care. If there are any concerns or questions about your treatment, please feel free to reach out to be directly. However, if there is an issue that you feel needs to be elevated, you have the right to report any concerns.
